For Donors
Before making your LPA, we encourage you to:
- Make a careful, considered choice on who to appoint as your Donee(s) and Replacement Donee (optional).
- Have a conversation with your intended Donee(s) and Replacement Donee (if any) on your wishes and preferences so that they can better support you in the event you lose mental capacity.
Making your LPA
All
LPAs must be made and submitted in OPGO via
Singpass
login. Steps on how to make your LPA can be found here.
You
may submit a hardcopy form only if you fall under the following exceptions as
set out in
Section 11(2a) of the Mental Capacity Act:
a. The Donor/Donee/Replacement Donee is unable to use OPGO due to his/her physical disability or other circumstances;
b.
OPGO is unavailable;
c. The Donor does not have an electronic device to receive SMS to create and access his Singpass account/National Digital Identity (NDI);
d. The Donor/Donee/Replacement Donee is not eligible for a Singpass
account/NDI.
The acceptance of hardcopy LPAs is subject to the Public Guardian’s approval. Please use the 2022 version of the hardcopy LPA form in your submission; older versions of the form may be rejected by OPG.
Please note that persons with an existing Court Order granting powers to an individual under the previous Mental Disorders and Treatment Act will not be able to make an LPA granting the same powers to a Donee.
Registered LPAs
All hardcopy and softcopy registered LPAs made before or after OPGO launch, are made available on OPGO. You and your Donees may log in to OPGO via Singpass to view a copy of the registered LPA.
Replacement Donees will only be able to view the registered LPA when they become the Donee. If you have appointed a Replacement Donee, you are encouraged to share a copy of your LPA with him/her via the 'send' function on OPGO for reference.
In the event both you and your Donee(s) lose mental capacity, the Replacement Donee (if any) must inform OPG so that OPG can update the LPA to allow the Replacement Donee to step in.
